Effective July 1, 2023: Express Scripts National Preferred Formulary Exclusion List Changes
This list shows the excluded medications not covered on the Express Scripts National Preferred Formulary beginning July 1, 2023. In most cases, if you fill a prescription for one of these drugs, you will pay the full retail price. It is encouraged to discuss with your provider the consideration of writing a new prescription for one of the preferred alternatives.

This formulary is the list of drugs included in your prescription plan. Inclusion on the list does not guarantee coverage. This list is not a complete list of over-the-counter (OTC) products and prescription medical supplies that are on the formulary. The only OTC products and medical supplies that appear on the list are in contracted classes.

This formulary is the list of drugs excluded in your prescription plan. The excluded medications found on this list are not covered on the Express Scripts drug list. In most cases, if you fill a prescription for one of these drugs, you will pay the full retail price. If you are currently using one of the excluded medications, it is advised to discuss with your doctor the consideration to write a new prescription of one of the preferred alternatives. Additional covered alternatives may be available.
