Welcome to OAOC! We are happy to have your student enrolled with us. Enclosed you will find enrollment paperwork essential to their success in the upcoming school year. Please carefully read, sign and return each form.

Acceptable Use Policy - signature from both parent/guardian and student
Registration card - please update all information for accurate communication
Student Health History Update
Medication Request - if you wish your student to receive medications while at school
FERPA form - only sign and return IF you wish to opt-out
Acknowledgment of Attendance Notes/Medications at School Policy
School Lunch form or Direct Certification letter - if your student will be eating breakfast or lunch while at OAOC
